School Based Health Center

The Oakdale School-Based Health Center (SBHC) is located in Allen Parish on the campus of Oakdale Middle School. The 2,100 square foot clinic is easily accessible for elementary, middle and high school students in Allen Parish. Students can visit the health center and parents can request that their child receive immunizations or check-ups by communicating information to the Medical Assistant or Nurse Practitioner on staff. All students must have a current consent and enrollment form on file at the health center to be seen by the Nurse Practitioner. Consent and enrollment forms are sent home at the beginning of the school year; however, parents may sign their child up for services at any time during the school year. Co-payments are waived for all students and faculty/staff enrolled at our SBHC. Commercial insurances including Medicaid and Medicare are accepted. Students who are uninsured can also receive care with a consent and enrollment form on file with the health center. Any medications prescribed for your child may be filled through Access Health Louisiana Pharmacy and delivered to your home. We also work with nearby Walgreens and CVS pharmacies and can have most prescriptions filled at a discounted rate.
 

At the Oakdale School-Based Health Center (SBHC) we are now able to provide medical and behavioral health services to all Allen Parish School faculty and staff through Telehealth with Access Health Louisiana. Telehealth services for faculty and staff are by appointment only. Consent form can be found by CLICKING HERE . Students needing emergency care after hours, please call 9-1-1. For students needing to continue their care throughout the summer or on school breaks, visit Dr. David Guillot, Jerry Stallings, FNP, or Randalla Reynolds, NP at the Woodworth Community Health Center (CHC) located at 9372 Highway 165 South in Woodworth (just 20 minutes from Oakdale). The Woodworth CHC is a member of the Access Health
Louisiana Network. Students registered with Access Health Louisiana through the Oakdale SBHC do not need to re-register for care when seen at the Woodworth CHC. For appointments or information, please call (318) 484-9588.
